Point and Click on the Following Pages:

1. Select the Building, Floor and Room needing repair.

2. Select Available Services: Always choose Facilities Management (Do Not Use “Create Your Own”).

3. Select the problem from the major categories:

  • Carpentry
  • ECC (Room Temperature)
  • Electrical
  • Fabrication
  • Housekeeping Including, Regulated Medical Waste Pickup
  • HVAC
  • Minor Maintenance Including, Shredding
  • Painting
  • Plumbing

 

4. Select the details of the problem.

Note: "Make as Was" Repair Service Requests - Repairs, building maintenance, grounds and housekeeping work orders are not billed to the requester. These work orders are “to make as was,” that is, returning the buildings and/equipment to its original or operating condition. This does not include Minor Services and Room Scheduling.
